Services: Extending primary care directly into the home by Community Paramedics, nurses and pharmacists.  Clinical monitoring, chronic disease management, post- surgical/ICU discharge monitoring, ambulatory detox, crisis response with the Hope Center, case management and care coordination, medication administration, clinical interventions (wound care, tracheostomy, ostomy, phlebotomy), staff MIRA bus. Spanish services.

Eligibility: No requirements, appointments are necessary and can be scheduled by calling or submitting a Referral Form. Serves Eagle County.
