Home and mobile health care

Home Key Health

Services: Provides home health and home care services, along with remote patient monitoring and primary care support. Includes advanced nursing with lab specimen collection, IV therapy, wound care, chronic disease prevention, post hospital discharge care, care coordination for rural families and telehealth facilitation with physicians.

Cost: Takes all insurance and sliding scale available.

Eligibility: Anyone in need of services. Physician referral is not required.

Eagle County Paramedic Services

Services: Extending primary care directly into the home by Community Paramedics, nurses and pharmacists.  Clinical monitoring, chronic disease management, post-surgical/ICU discharge monitoring, ambulatory detox, crisis response with the Hope Center, case management and care coordination, medication administration, clinical interventions (wound care, tracheostomy, ostomy, phlebotomy), staff MIRA bus. Spanish services.

Cost: Free

Eligibility: No requirements, appointments are necessary and can be scheduled by calling or submitting a Referral Form. Serves Eagle County.

The Elephant Circle

Services: Birth Doula Services; Medicaid, low cost and free doulas. Spanish services.

Cost: Clients who have Medicaid can receive services at no cost to them. We also have doulas that can provide services for free or low cost for non-Medicaid clients. Clients who do not have Medicaid should reach out to see what the cost would be for them as it varies widely.

Eligibility: Any pregnant person in Colorado. Serves Pitkin, Eagle & Garfield counties.

MIRA Bus

Services: We connect people to basic health education and screenings, support in applying to public assistance programs (Like Medicaid, SNAP, and the Benefit Recovery Program), insurance enrollment, food resources, immunizations, workforce development, coordination with childhood programming, perinatal support, behavioral health resources, and more. Additionally, MIRA hosts monthly community health education events. Spanish services.

Eligibility: No requirements, No appointments necessary. Serves Eagle County.
