Services: This HRSA-managed tool connects individuals with Ryan White-funded medical providers offering Core medical services; Primary HIV/AIDS medical care, Dental care, Early intervention services, Medical case management & treatment adherence, Mental health care, and Support services; Transportation to appointments, legal assistance, Insurance enrollment & premium support, Psychosocial support and outreach. Cost: Services are low-cost or free for eligible individuals. Care and medications are provided on a sliding-fee basis, with coverage for uninsured or underinsured low-income people.

Eligibility: To access services, you must: Be diagnosed with HIV/AIDS, Have low income, and Be uninsured or underinsured (your insurance may not fully cover needed care). Contact a provider directly to confirm eligibility, schedule an appointment, and discuss documentation. Serves Pitkin, Eagle & Garfield counties.
