Sagebrush Recovery

Services: Outpatient substance use and co-occurring mental health support via clinical counseling and peer recovery coaching. Individual sessions, recovery groups, care coordination, relapse-prevention planning, resources & referrals (housing, employment, medical/behavioral health), family/support-system education, and overdose prevention education with linkage to naloxone.

Cost: Medicaid accepted (all regions). Self-pay available; sliding scale for eligible clients. Call your insurance for coverage and deductible (cost) before appointment then call center to see what you qualify for.

Eligibility: Adults (18+) seeking support for substance use and/or mental health – no referral required.

Hours: Hours and location vary as we are a widespread team often found working in the field or remotely. To inquire about signing up for support, please call us anytime at 719-341-2225 and we will connect within 24 hours.

Address: While our headquarters is located in Salida, we serve several rural mountain communities including Garfield, Pitkin, Eagle and Mesa Counties. Our services are provided through a hybrid in-person and Telehealth model.
