We are hiring a full-time Regional Health Connector (RHC) for the WMRHA team! The RHC strengthens collaboration among healthcare, behavioral health, and community-based organizations to support WMRHA’s mission of building an integrated healthcare system for optimal health in the West Mountain region. Serving Garfield, Pitkin, Eagle, and Summit counties, this role implements strategies to improve care coordination, reduce barriers to access, and strengthen social determinants of health.
This position is ideal for someone who is a skilled relationship builder, passionate about community health, and experienced in public health and social determinants of health. The RHC will work closely with WMRHA leadership, regional coalitions, and partner organizations to identify gaps, align resources, and foster equitable, community-driven solutions. Strong facilitation skills and the ability to bring diverse partners together toward shared goals are essential
