Valley Settlement – Equipo de Apoyo Familiar / Family Support Team

Services: Case Management & Community Services. Wraparound services; Family Navigators conduct home visits, assist families in times of crisis, and provide one-on-one support to connect families with the resources they need to thrive. This can include support with healthcare, food, transportation, government assistance, etc. Service in English and Spanish

Cost: Free

Eligibility: Currently accepting referrals for individuals/families with children ages zero to five years old.

Benefits in Action

Services: Assistance navigating and applying for government benefits. SNAP, Medicaid, Medicare, Connect for Health, Social Security, unemployment, LEAP, and more!

Cost: Free

Eligibility: Anyone that lives in Colorado, regardless of citizenship.

Eagle County Public Health and Environment; Healthy Families

Services: Nurse Family Partnership (NFP) and Family Connects (both are nurse home visiting programs), and HCP (a care coordination program for children and youth with special health care needs).

Eligibility: NFP is available to first time moms who meet income eligibility. Family Connects is available to all Eagle County residents, without any other eligibility requirements. HCP is available to all Eagle County residents who are caring for children 21 and under with a special health care need.

Cost: All programs are offered at no cost.

Pitkin County Senior Services

Services: Activities and events offered throughout the week.

Cost: Free

Eligibility: Must be a senior. Walk-ins welcome at the Senior Center during open hours.

Grand River Medical Center – Meals on Wheels

Services: Hot and cold meal home delivery

Eligibility: Elderly, disabled, homebound, and recovering individuals living in New Castle, Silt, Rifle, or Parachute/Battlement Mesa. Sign up by calling 970-625-6215

Pitkin County Senior Center: Lunch Program

Services: Hot lunch, nutritious grab n go meals in a community setting. Home delivered meals.

Eligibility: Seniors, make a reservation by noon at least 24 hours in advance by calling 970-429-6161. For home delivered meals call 970-920-5432.

Cost: 60 and older the recommended voluntary contribution is $4 per meal, under 60 is $9 per meal.

Eagle County Senior Services

Services: Hot community lunch, Home delivered meals

Cost: 60 and older the recommended voluntary contribution is $4 per meal, under 60 is $14 per meal.

Eligibility: Eagle County residents 60 and over. Make a reservation by noon at least 24 hours in advance of the meal by calling the correct site location.

Garfield County Senior Programs

Services: Hot lunch and nutritious grab and go meals in a community setting. Delivery available call for details.

Eligibility: Seniors, make a reservation by noon at least 48 hours in advance of the meal by calling the correct site location.

Cost: 60 and older the recommended voluntary contribution is $3 per meal, under 60 is $10.25 per meal.

Valley Meals and More

Services: Home delivered meals, grocery store pick-up and delivery. Safety checks and friendly visits.

Cost: No cost; donations accepted

Eligibility: Elderly (60 years and older) and homebound (due to acute or chronic conditions) in the eastern Garfield County area, from West Glenwood to El Jebel and up the Crystal Valley.
