Care Coordination

Vail Valley Charitable Fund (VVCF)

Services: Direct Aid: Helps local individuals and families facing medical crises or long-term illness with grants to cover medical bills, rent, utilities, or other essential needs. Eagle County Smiles Program: Provides free or reduced-cost dental care for uninsured children and youth in our area of service. Eagle County Grins Program: Offers low-cost dental services for uninsured adults. Vail Breast Cancer Group (VBCG): Supports women in Eagle County who are undergoing breast cancer diagnosis or treatment through financial aid, mammogram funding, and community support. Spanish Outreach: Bilingual (English/Spanish) assistance to help applicants navigate forms and connect with our programs. Community Referrals: Connects clients with additional resources such as mental health, housing, transportation, or food assistance programs. Volunteering and Events: Opportunities for community members to get involved in fundraising events like the LG Tri or Bowling for Boobs and to give back.

Cost: All applications and the application assistance for our programs are free of charge.

Eligibility: Must live or work in our area of service (East Vail to Dotsero, including Minturn & Red Cliff) for more than one year. Must be facing a medical hardship or financial difficulty related to a health issue. Applicants provide documentation such as physician statements, medical bills, rent agreement / mortgage statements, and proof of income. For EC Smiles and Free Screening Mammogram programs, specific applications are required (available on the website).

Beneficent

Services: Learn how to pay for long-term care. Free Consultations – Certified Medicaid Planners offer free consultations for those who want to explore their options for paying for long-term care—specifically utilizing Medicaid or VA Aid and Attendance. Completing Applications – Beneficent Case Managers will guide you through the application process, avoiding common mistakes and ensuring success until approval. Lump Sum Report – If a current Medicaid or VA applicant sold a home or received a lump sum of cash and is worried about losing their long-term care benefits, we outline strategies to preserve the cash and retain benefits. Free Learning Events – Complimentary learning events about Medicaid and VA Aid & Attendance benefits for senior care providers and industry partners.

Cost: Free 1-hour consultation. During the consultation, we will discuss our flat fee or provide instructions on how you can qualify for Long-Term Care Medicaid or VA Aid & Attendance, if you want to apply on your own.

Eligibility: Must be over the age of 65 or a disabled adult. An applicant must earn less than $9,000 in monthly income; spouse income is not included. Require assistance with at least 2 activities of daily living – bathing, dressing, eating, or using the toilet.

Division of Vocational Rehabilitation (DVR)

Services: Employment services for individuals with disabilities.

Cost: Free

Eligibility: Call (970) 945-1042 to schedule an appointment/intake or apply online. Must have a disability to be eligible for services.

Mountain Valley Developmental Services

Services: We provide support and services to children and adults who have intellectual and developmental delays and disabilities in Garfield, Pitkin, Eagle, and Lake Counties.  We are an Early Intervention Evaluator and an Early Intervention Broker.  We are also a Provider Agency for the Children’s Extensive Support Medicaid Waiver, the Supported Living Services Medicaid Waiver, and the Developmental Disabilities Medicaid Waiver. Our services cover the life span and are provided through a variety of programming options.

Cost: Free, Call for an appointment.

Eligibility: Must meet state determined criteria for Early Intervention and/or as a person with intellectual and developmental disabilities.

Eagle County Paramedic Services

Services: Extending primary care into the community and addressing an individual’s ability to achieve personal health goals and positive health outcomes, by addressing whole-person health and connecting healthcare resources. For example; clinical monitoring (vs, sp02, EKG, Wt, INR), telehealth facilitation, phlebotomy, chronic disease management and education, medication management, nutrition consult, dementia care coordination, long term care planning, home medical care facilitation, medication administration, clinical interventions (wound, tracheostomy, ostomy, foley care).

Cost: Free

Eligibility: No requirements, appointments are necessary and can be scheduled by calling or submitting a Referral Form.

Eagle County Public Health and Environment; Healthy Families

Services: Nurse Family Partnership (NFP) and Family Connects (both are nurse home visiting programs), and HCP (a care coordination program for children and youth with special health care needs).

Eligibility: NFP is available to first time moms who meet income eligibility. Family Connects is available to all Eagle County residents, without any other eligibility requirements. HCP is available to all Eagle County residents who are caring for children 21 and under with a special health care need.

Cost: All programs are offered at no cost.
